Reporting Requirements

Worker’s Compensation

Employees are required to report work-related injuries, illnesses, and accidental or potential exposures as soon as possible to their supervisor. Supervisors are required to complete the electronic University of Minnesota First Report of Injury Form (E-FROI) online within 8 business hours upon first knowledge. This is required to ensure appropriate follow-up and eligibility for Workers Compensation coverage.

For additional information on reporting Workers Compensation Related Injuries, see Human Resources' Workers Compensation Policy.

IBC Protocols

If an accidental or potential exposure has occurred during work on an Institutional Biosafety Committee (IBC) protocol, the supervisor must also submit an Incident Report Form to the IBC
