Frequently Asked Questions

Why am I getting e-mail reminders to complete an immunization, fit test, audiogram, etc.?

You are receiving reminders because you have been enrolled in one of the Programs found on our Program and Instructions webpage. You may have been enrolled for the following reasons:

  • You may have been added to an Institutional Biosafety (IBC) protocol or an Institutional Animal Care and Use Committee (IACUC) protocol. You may log-in to the Occupational Health Portal to see whether you are enrolled in a protocol (scroll to the bottom of the page and click on the “Protocol Information” tab).
  • You may have been automatically assigned requirements because of your work responsibilities (e.g. you will be providing animal support services for research animals or client-based animals; or you may be providing patient medical services; or you may require protection from air-borne or noise hazards). Contact your supervisor for further information.
  • You may need access to areas of the University that have requirements in place (e.g. areas where research animals are housed or the Biosafety Level 3 facilities). Contact your supervisor for further information.

How do I find my requirements?

How do I submit my immunization records?

Visit the BOHD Submit Documents webpage.

How do I schedule an appointment to complete medical requirements?

Visit the BOHD Clinical Services webpage. Note that before completing a fit test, you must be cleared to wear a respirator and complete training. See the Respiratory Protection Program webpage for more information.

How do I complete my training and what to do if I run into issues?

Log-in to the Occupational Health Portal and click on the training links. Visit the HSRM Training website to troubleshoot training issues.

How do I stop the e-mail reminders from [email protected]?

If you have moved on to a new position or are no longer at the University, please e-mail [email protected]. Note: If you are listed on an IBC/IACUC protocol, BOHD is unable to remove you from the protocol. You must contact the PI and request that they remove you from the protocol. You may log-in to the Occupational Health Portal to see whether you are still enrolled in a protocol (scroll to the bottom of the page and click on the “Protocol Information” tab).
